Plan Care Australia is a Melbourne-based NDIS business that was born out of the founders’ desire to leverage their diverse skills, training and experience to develop a company that helps to improve the lives of people participating in the NDIS. We offer a range of services, including plan management, support coordination, therapy and assessment services, and home modifications. We are committed to providing quality care and support that meets the needs of each individual we work with. Our vision is for all people with disability to be able to access the necessary supports they need to live life their way.
